Matt Serlo, MPT

Matt is the co-owner and founder of Five Star Physical Therapy. He received his Masters from Gannon University and is the former Administrator of Healthsouth Sports Medicine Center in Jacksonville Beach, FL. He also served as director of education and regional coordinator of the Golf Performance Program for 7 years. Matt has over 20 years experience as a physical therapist in Orthopedics & Sports Medicine and is an esteemed lecturer with the ACE institute, educating rehabilitation specialists and golf teaching professionals the science of performance enhancement and injury prevention. He has worked closely with Dr. Andrews in Birmingham, AL and has been instrumental in several research projects involving the biomechanics and injury implications of the golf swing and throwing motion of athletes at the American Sports Medicine Institute. Matt has extensive experience working with athletes of all levels including NFL, PGA, LPGA & Champions tours. He is also a member of the American Physical Therapy Association and is affiliated with the PGA Tour Golf Academy at TPC Sawgrass in Ponte Vedra Beach and World Golf Village in St. Augustine, Fl. Matt currently resides in Neptune Beach, FL with his wife and their two children Jack and Charlotte.

Larry Feldman, PT

Larry received his B.S. degree in Physical Therapy from Ithaca College/Albert Einstein Medical Center in NY where he also played four years of Varsity Basktball. Larry has over 20 years experience as a Physical Therapist specializing primarily in outpatient orthopedics & sports medicine. He founded Complete Care Physical Therapy which operated in 4 locations on Long Island, NY, as well as the medical networking entity Rehabilitaion Networks of America (specializing in retaining managed care contracting for healthcare professionals). He has partnered in the large Health & Racquet club Sportset in Rockville Centre, N.Y and is now a co-owner of Five Star Physical Therapy. Larry actively coaches softball & baseball in the Ponte Vedra Athletic Association & provides lectures on mulitple topics in the field of Physical Therapy. He also enjoys teaching both coaches & players about injury prevention and wellness in the young athlete. Larry now resides in Ponte Vedra Florida and lives with his wife and two children Brianna and Bryce.

Jenna Roberts, DPT

Jenna joined Five Star Physical Therapy in August 2016. Jenna was born and raised in New Jersey. She participated in dance growing up, which sparked her passion for movement and science. She moved back to Jax after graduation and likes to spend her time biking and relaxing on the beach, working out, cooking, and trying new restaurants in the area.

Mark Kosier, DPT, RCMT

Mark graduated from University of Florida in 1978, earning Bachelor’s Degrees in both Neuroscience and Physical Therapy. He has since completed a two year study program under Dr. Mariano Rocabado and became a Rocabado Certified Manual Therapist. Mark completed a fellowship from the American Academy of Orthopedic Manual Therapy and earned his transitional DPT through the University of St. Augustine in 2010. With over 25 years of experience in physical therapy, Mark has become an esteemed lecturer across the United States in the areas of orthopedic manual therapy and has special clinical interest in treating head, neck, and facial pain. A native to Detroit, Mark is an avid sailor and enjoys camping with his wife and four kids.

Lizelle Lapuz, PTA

Liz joined our staff in November 2016. She earned her Bachelor of Arts degree in Interpersonal and Organizational Communication from the University of Central Florida in 2009. Shortly after graduating, Liz enlisted into the United States Air Force as an Emergency Manager and got to travel all over the world. After four years of being away from sunny Florida, Liz finally moved back to her hometown of Jacksonville and transitioned into the Florida Air National Guard. She then went back to school to earn her Associate of Science degree in Physical Therapist Assistant from Keiser University. In her free time, Liz enjoys hanging out with family and friends, watching sports, eating delicious food, and going on spontaneous weekend trips with her boyfriend, Steve.

Clark Williams, PT

Clark graduated from Virginia Tech in 1993 with a B.S. in Animal Science and in 1984 with a B. S. in Agricultural Economics. For seven years Clark worked for what is now Bank of America as a Commercial Account Executive during which time he attended University of Virginia and graduated from Virginia Bank Management School in 1989. Clark made a career change in 1990 and decided to pursue becoming a physical therapist. He graduated from Shenandoah University in 1995 with a Masters in Physical Therapy. He opened Williams Physical Therapy in February of 1997. He is a Certified Strength and Conditioning Specialist and holds a Degree in Pedorthics from Oklahoma State University-Okmulgee. Clark is certified as a Level I Clinical Instructor (CI) from the American Physical Therapy Association. Clark’s post graduate training has been extensive in the areas of orthopedics, manual therapy, sports medicine, Mckenzie evaluation and treatment of spine disorders, orthotic fabrication and industrial rehab. Clarks continues to work closely with all the coaches and athletes in the surrounding local high schools as well. Clark is married to Josee, who is also a PT with WPT and together they have two children who are active in sports. As a family they enjoy horseback trail riding and working with horses in general. Clark enjoys trying to stay fit with exercise activities such as Crossfit. He also enjoys road bike riding. He loves listening to contemporary Christian music, 70’s and 80’s and attends Cliffview Church of God.

Ian Stevenson, DPT

Ian graduated from Duquesne University with a Bachelors of Science in Biology and a Bachelors of Science in Health Sciences in 2009. He then subsequently graduated with his Doctorate in Physical Therapy, also from Duquesne University in 2011. After graduation Ian began his professional work in Southern Maryland at an outpatient orthopedic clinic where he was previously at for the last four and a half years before relocating to Galax. From his vast experience in orthopedics, Ian has continued to further his knowledge in several manual therapy techniques, including McKenzie Mechanical Diagnosis Therapy, Maitland techniques, Mulligan techniques and has also been certified in the use and application of the Functional Movement Screen. With the proximity to several high schools, colleges and minor league teams, Ian has experience in treating a multitude of sports related injuries but still loves to treat injuries ranging from total joint arthroplasties to complex rotator cuff injuries and surgery rehabilitation. Ian is originally from Connecticut, where he grew up his whole childhood through high school, where he then moved to Pittsburgh and after completing school, moved to Southern Maryland where he met his fiancé. Being from Elkin, NC, his fiancé and himself have come back to the area in order to establish their own family and roots while being close to family. In his spare time, he loves being outdoors whether it be fishing, hiking, kayaking or just spending time expanding his small and newly established organic hobby farm in Elk Creek, living in his self-made tiny house on wheels with his fiancé.

She enjoys working out and challenging herself as she has conquered exercise programs such as “Insanity”, Ripped 60, Tapout XT and Insanity Asylum. She enjoys cooking and trying new recipes. She also speaks French, as this is her mother tongue.

Jodi Bridges-Lemoine, MPT

Ginny Yuratich, MSPT

Ginny is from Arlington, Texas and is one of six children. Following in her parents’ footsteps, she graduated from LSU with a Bachelor of Science degree in Kinesiology. Ginny attended graduate school at Regis University in Denver, Colorado and graduated with a Masters of Science in Physical Therapy in 2001.

Ginny Yuratich, MSPT

Ginny is from Arlington, Texas and is one of six children. Following in her parents’ footsteps, she graduated from LSU with a Bachelor of Science degree in Kinesiology. Ginny attended graduate school at Regis University in Denver, Colorado and graduated with a Masters of Science in Physical Therapy in 2001.

Rachel Clements, DPT

Rachel, a lifelong resident of Chalmette, LA, graduated from LSU with a Bachelor of Science degree in Kinesiology. She attended graduate school at LSU Health Sciences Center where she received her Doctorate of Physical Therapy degree. At LSU she was awarded the Dean’s List and Chancellor’s List Awards and was a recipient of the Board of Supervisors scholarship.
